Tennessee Department of Education
"Home School Student Attendance" form

The following example of the attendance form is dated for the 2001-02 school term but this form has remained essentially the same for years. A .pdf version of this document can be found at: http://tennessee.gov/education/homeschool/doc/0607attendance.pdf

Please remember that Tennessee Code Section 49-6-3050(b) states:

(2)“Maintenance of attendance records, subject to inspection by the local superintendent and submission of these records to the superintendent at the end of each school year;”
(3)”Instruction for at least four hours per day for the same number of instructional days as are required by state law for public schools;” [currently 180 days—K]

The code doesn't say which 180 days of the year, it doesn't exclude holidays or weekends or summers. It also doesn't say which 4 hours of the day.

There are no specific instructions on the form as to how to fill it in. In marking the form I have known homeschoolers to use "P" or "X" marks or coloring it in for days they had school. I've never heard of the form being rejected. I would encourage you to double and triple check the form to be sure it's accurate.

Obviously, the form below is quite dated now but the format remains the same. Download the current one here.
