Intent to Home School Forms

8/1/06 I had a conversation with Nancy Dill the woman in charge of these homeschooling files today. She tells me that they understand that the form is asking for more than the law requires. She maintains that the information is helpful to the school system as they track students, that the local school receives a copy of this three part form that doesn't include that personal information and that if a parent chooses not to provide the information MNPS may call requesting it but will not hound parents for the information but file the form away.

6/28/06: I'm throwing these up on the web immediately for folks to review. I'll follow up later with specific information about what is wrong with these forms (SSN, DOE authorizing CRS's, School of Zone). For now please consult the actual code about what information you are legally required to provide. That can be found at http://TnHomeEd.com/49-6-3050 (b).

These forms are dated January 2006 and I received them from MNPS on 6/28/06.

These forms are in .jpg format and may take a few moments to completely load depending upon your Internet connection speed.
